Two people with very similar needs can want completely different things. That is why a personalised care plan starts with you as a person: your routines, your preferences, the things that make a good day, and the goals you are working towards.
We build every plan together with the person we support and the people who know them best. Nothing about your care is decided without you. Your plan belongs to you, and you have the final say.

How we build your plan, together
We listen first
We start with a conversation, not a form. We take time to understand you, your goals and what matters most to you.
We plan together
We co-produce your plan with you and, where you want, your family and the professionals involved. You sign it off.
We agree the detail
We confirm exactly what support looks like, who provides it and when, and how we will know it is working well.
We deliver and stay close
A consistent team puts the plan into practice, and we check in regularly to make sure it still fits.
We review and adapt
We review your plan on a regular basis, and whenever things change, so your support always reflects the person you are today.

Person-centred

Least restrictive
We support you in the way that protects your independence and freedom as fully as possible. Where someone may not be able to make a particular decision, we follow the principles of the Mental Capacity Act and act in their best interests.

Your plan is a living document. We review it regularly and update it as your needs and goals change.

FAQs
Common questions
Who writes the care plan?
We write it together with you. We may also involve your family and the professionals supporting you, with your agreement, but you have the final say.
How often is the plan reviewed?
We review your plan on a regular basis and whenever your needs or circumstances change, so it always reflects what you need now.
What if I cannot make a particular decision myself?
Where someone may lack the capacity to make a specific decision, we follow the Mental Capacity Act, involve the right people, and make decisions in that person’s best interests and in the least restrictive way.
Can my family be involved?
Yes, as much or as little as you would like. Many people choose to involve a family member or an advocate, and we welcome that.
How does this work with a local authority assessment?
If your care is arranged through the local authority, your plan works alongside your Care Act assessment and support plan. We are happy to work closely with your social worker.
